In accordance with Borough of Morris Plains and N.J.S.A. 15A:3A-1 et seq, as a condition of volunteer service, the Borough of Morris Plains requires background checks on all individuals who will be working with children. These background checks are good for three years and will not be given to anyone under the age of 18years. Please contact the Borough Clerk’s office to obtain a consent form.

Behavioral Policy(Required)

Both my teen and I have reviewed and agree to the Behavior Policy, and submission of this form constitutes our signed agreement.

Morris Plains Teen Center Behavioral Policy

The goal of the Morris Plains Teen Center (MPTC) is to provide a safe, social, inclusive and welcoming environment for all of its members.

The MPTC believes a positive environment is essential for our community. All MPTC members have the right to be treated with respect and compassion while having fun.

With rights come responsibilities. Everyone must work together to achieve a positive environment at MPTC, therefore, all members have the responsibility to practice the following:

Responsibility:
*Behave my very best at all times.
*Accept responsibility for my decisions, actions, and their consequences.
*Have the courage to apologize when I make a mistake that is wrong or unkind toward others.
*Demonstrate that I have PRIDE in being a citizen of Morris Plains by my actions.

Respect:
*Show that I have respect for myself by my actions.
*Treat others (peers, chaperones, supervisors) with respect and courtesy.
*Demonstrate respect and tolerance of others regardless of differences in gender, race, ethnicity, religion, physical ability, and any other differences.

Citizenship:
*Show respect to the property, building/and or grounds
*Show respect to other people’s belongings
*Clean up after yourself

Digital Citizenship:
*My phone will be placed in the pocket holder during the event
*I will not use my phone to take pictures or video of any other member
*I will not have my phone out during an event.

Procedure for Behavioral Offense is as follows:

1st offense-verbal warning

2nd offense-call home and removal from event

3rd offense (carried over from 2nd offense)-member will be sent home and prohibited from attending the next event.

A re-entry meeting with parents, TC Coordinator & TC Supervisor will be held.

4th offense-TC member will be asked not to return for the remainder of the year

I agree to pay $25 membership fee online or by check and and I understand that if I pay online, I will be separately charged a processing fee by our online payment provider, MuniciPAY.

MuniciPAY accepts credit or debit cards. You will be assessed a nonrefundable 2.65% credit card fee by MuniciPAY of the total amount of the charge, or $3.00, whichever is greater.

I understand that events will have an additional $10 fee to be paid in cash the night of or prior to the event, as communicated throughout the year.
